New Patient Information

We're accepting new patients 18 and older. Here's everything you need to know before your first visit — forms, what to bring, insurance, and what happens once you're in the door.

Starting with a new doctor can feel like a hassle — but we've tried to make it as simple as possible. Here's exactly what to expect from the moment you decide to schedule through your first visit at West Atlanta Primary Care.

How to Schedule Your First Appointment

You can book online anytime through the healow app or website, or call our office at (678) 401-4597 during business hours, Monday through Friday, 8:00 AM to 5:00 PM. If you need to be seen urgently, mention that when scheduling — we hold same-day slots for acute concerns whenever possible.

What to Bring to Your First Visit

Please bring a valid photo ID, your current insurance card, a list of medications and supplements you take (including dosages), and any relevant medical records or recent test results from previous providers if available. If you're transferring care, it's helpful — though not required — to request your records be sent ahead of your visit.

New Patient Forms

To save time at check-in, we recommend completing your new patient forms before your visit. This includes the New Patient Registration Form, Medical History Form, HIPAA Acknowledgment, Financial Policy & Assignment of Benefits, and Consent for Treatment. All forms are available on our Clinical Forms page and can be filled out digitally or printed.

What Happens at Your First Visit

All new patients are seen in person for their first appointment to properly establish care — this lets Dr. Desai take a complete medical, family, and social history and perform a baseline physical exam. Depending on your age and health history, we may order baseline labs at this visit. Plan for a somewhat longer appointment than a typical follow-up, since we're building the foundation for your ongoing care together.

Insurance We Accept

We accept most major commercial insurance plans, Medicare and Medicare Advantage plans, and TRICARE for military and veterans. For a full list of accepted insurance providers, visit our Insurances page, or call our office and we'll verify your specific benefits before your visit.

Questions Before You Book?

Our WAPC Assistant chatbot is available 24/7 on our website for quick questions about scheduling, forms, or insurance. For anything more specific, our front desk team is happy to help by phone during business hours.
